Avedis Donabedian Foundation for the Improvement of Health Care Quality (NIF G59026716), hereinafter referred to as FAD ("we", "our" or "us") operates the online “SAFEST platform” (hereinafter, "platform") and its associated mobile application "SAFEST Patient App" (hereinafter, "the Application"). 

This page informs you of our policies regarding the collection, use, and disclosure of data when you use our platform and/or the associated Application. It also explains the choices you have associated with that data. 

Collection and Use of Information 

Personal Data:  We collect personal data solely for the effective operation of the study. We prioritize minimizing data collection and ensure that only essential information is gathered: 

  • Platform: Personal information such as your name, contact details (email and phone number) is collected on the platform. 

  • Application: The App only requires your email address for login purposes. No additional information that could directly identify you appears within the App. 

Survey Responses: We collect health information, patient experiences, and quality-of-life data related to your surgery to support research goals. 

Usage Data: We do not collect data such as your device's Internet Protocol address (for example, IP address), browser type, browser version, pages visited, the time and date of your visit, the time spent on those pages, unique device identifiers, or other diagnostic data. 

Cookies and Tracking Technologies:  Our platform and Application do not use cookies or similar tracking technologies. Additionally, no information is stored on your device during use. 

Data Disclosure 

Your data will not be shared with third parties outside of the SAFEST research team. Anonymized or de-identified data may be included in open-access publications to further scientific knowledge. 

Data Security 

Personal data such as your name and email address collected on the platform is stored on secure servers with industry-standard encryption protocols. 

Access to this data is restricted to authorized personnel within the SAFEST research team and is protected by multi-factor authentication. 

The Application only requires your email address for login purposes. No other personal data is collected or stored in the App. 

Links to Other Sites 

Our paltform and Application may contain links to other sites that are not operated by us. If you click on a third-party link, you will be directed to that third party's site. We strongly advise you to review the Privacy Policy of every site you visit. 

We have no control over and assume no responsibility for the content, privacy policies, nor do we assume any responsibility for the content, privacy policies, or practices of third-party websites or services. 

Privacy of Minors 

Our platform and Application are not intended for individuals under the age of 18 ("Children"). 

We do not knowingly collect personally identifiable information from individuals under the age of 18. If you are a parent or guardian and you are aware that your child has provided us with Personal Data, please contact us. If we become aware that we have collected Personal Data from children without verification of parental consent, we will remove that information from our servers. 

Changes to This Privacy Policy 

We may update our Privacy Policy from time to time. We will notify you of any changes by posting the new Privacy Policy on this page.
